IMBH’s case management and care coordination model is designed to advance whole-person, patient-centered care by seamlessly integrating behavioral health services with medical care and medical case management. Recognizing the challenges members face in navigating a complex healthcare and insurance landscape, our experienced team serves as a trusted guide—connecting individuals to the right providers, closing gaps in care, and ensuring services are aligned across the continuum. Through proactive coordination and individualized support, IMBH helps prevent illness progression, enhance care engagement, and drive improved health outcomes.

Identifying Target Population

Modeling guidance with health plans to help identify high-utilizers, complex and chronic cases, and those patients with co-morbid medical conditions.

Peer Support

Peer support and other wrap-around services help connect members with local resources to help manage their behavioral health recovery.

Elder Care Case Management

Patients 65 years old and older may need extra support to manage their changing circumstances.

Patient Monitoring

Measurement-based care tools via web and app report patient outcomes to IMBH allowing early detection of any decline.

First Episode Psychosis

First Episode Psychosis (FEP) programs provide early, coordinated intervention for individuals experiencing their first psychotic episode, combining psychiatric care, therapy, family education, and care coordination. Early identification and treatment are critical to reducing symptom severity, improving long-term recovery, and preventing disability and repeated hospitalizations.

Levels of Case Management Based on Acuity

IMBH team includes licensed clinicians, social workers, and community health workers to provide in-person, home, and telephonic case management depending upon need.
