Integrating behavioral health into primary care enables a coordinated, data-driven approach to whole-person care. IMBH leverages early screening, real-time collaboration, and evidence-based interventions to reduce avoidable acute care, improve treatment adherence, and drive measurable improvements in both behavioral and physical health outcomes.
Collaborative Care (CoCM) is a type of integrated care with a strong evidence base. For members with more than 2 chronic health conditions, IMBH implements Chronic Care Management (CCM) in addition to CoCM to deliver comprehensive and collaborative care.
Improve patient outcomes for mental health and substance use treatment delivered in primary care setting by having a IMBH clinician on-site.
On-site IMBH clinicians are able to facilitate psychiatric consultations for primary care providers treating mental health and substance use issues.
